HAZMAT ENDORSEMENT

OKLAHOMA RESIDENTS

PRIMARY

  1. U.S. Passport
  2. Certificate of Naturalization (INS Form N-550 or N-570)
  3. Unexpired foreign passport with I-551 stamp or attached INS Form I-94 indicating unexpired employment authorization.
  4. Driver’s license or ID card issued by a state, provided it contains a photograph or information such as name, address, date of birth, gender, height, and eye color.
  5. U.S. Military/Retire ID Card
  6. Military dependent’s ID card
  7. ID card issued by federal, state, or local government agencies or entities, provided it contains a photograph or information such as name, address, date of birth, gender, height, and eye color.
  8. Certificate of U.S. Citizenship (INS Form N-560 or N-561
  9. Permanent Resident Card or Alien Registration Receipt Card with photograph (INS Form I-151 or I-551)

SECONDARY

  1. Voter’s registration card
  2. U.S. Coast Guard Merchant Marine Card
  3. U.S. social security card issued by the Social Security Administration (other than a card stating it is not valid for employment)
  4. Original or certified copy of a birth certificate issued by a state, county, municipal authority or outlying possession of the United States bearing an official seal
  5. US Citizen ID Card
  6. Certificate of Birth Abroad issued by the Department of State (Form FS-545 or Form DS-1350)
  7. Native American tribal document
  8. U.S. Military Discharge papers DD214
  9. Civil Marriage Certificate
  10. U.S. Adoption Papers

Once TSA completes the security threat assessment, the applicant will be notified of the results by US Mail. Oklahoma Department of Public Safety will be notified whether the applicant may obtain or renew a hazardous materials endorsement.

LICENSES – TWIC

Transportation Worker Identification Credential (TWIC™)

The Transportation Worker Identification Credential (TWIC) is a vital security measure that will ensure individuals who pose a threat do not gain unescorted access to secure areas of the nation’s maritime transportation system.

TWIC was established by Congress through the Maritime Transportation Security Act (MTSA) and is administered by the Transportation Security Administration (TSA) and U.S. Coast Guard. TWICs are tamper-resistant biometric credentials that will be issued to workers who require unescorted access to secure areas of ports, vessels, outer continental shelf facilities, and all credentialed merchant mariners. It is anticipated that more than 1 million workers including longshoremen, truckers, port employees and others will be required to obtain a TWIC.

To obtain a TWIC, an individual must provide biographic and biometric information such as fingerprints, sit for a digital photograph and successfully pass a security threat assessment conducted by TSA. Pre-enrollment is recommended as it is designed to save the applicant time by enabling them to provide their biographical information and make an appointment for in-person enrollment.
